Death and dying are not something that we like to talk about, but knowing what your loved one's wishes are can make things so much easier. A care plan is built to help guide and support the dying and their families, and ensures the wishes of the dying are considered as their end of life approaches. There may be a time when the person will no longer be able to communicate; an Advance Care Plan ensures their wishes at the forefront of every decision. Advance Care Planning * 2 Hours + processing time. * Drafting Advance Statement * Reviewing Advance Decision to Refuse Treatment * Discussing POA & Legalities At the end you will have a bundle of documents to share with caregivers and medical team 2 x 60 minute sessions
